Please print out an application form. If you can not print PDF files, gather the following information:

• Name
• Mailing address
• Current address
• Home telephone
• Work telephone
• Cell phone(s)
• e-mail
• Why are you moving?
• Addresses and landlords for the past three years
• Current and previous employers
• Names and ages of all people who will occupy the premises, including applicant. Photo IDs will be required.
• Any smokers?
• Waterbed?
• Are you in the military?
• Do you use illegal drugs?
• Have you ever been convicted of a felony?
• Have you ever filed bankruptcy?
• Have you ever been evicted?
• When will you be available to move in?
• How long do you expect to stay at this residence?
• Current gross income per month?
• How many vehicles will be parked here?
• Any pets? Describe.

Note that affirmative answers to some of the above questions do not necessarily disqualify an applicant, but we might want to know more about the situation.

All persons over the age of 18 who expect to occupy the premises will be asked to sign the following:

I/we declare that the statements above are true and correct, and hereby authorize and instruct the landlords to obtain such credit reports and tenant screening reports as deemed necessary or prudent. I/we authorize and instruct any and all credit reporting agencies and tenant screening services to provide such reports to the landlord.
