- Only rarely is an expert available to assess a child's conscious state soon after an inflicted head injury.
- It may be difficult or impossible to ascertain when an infant was last neurologically normal when relying on a history provided by someone who may be, or may be associated with, the perpetrator.
- Uncertainties persist surrounding the issue of lucidity in infants who have been shaken.
